Got for $20 on local buy and sell site. Works great and lamp is barely used. It came with a very hazardous 2 prong cord connected but I took it off. I have two of thr lifeline BLB lamps

How well did it start with the two wire cord? Some single lamp fixtures were finicky about starting without a ground.

Power provided by Alabama Power, 120 volts 60 Hz. House is an LED free zone, tungsten and linear fluorescent only.

The lamp had to be touched in a wiping motion to make it light. Started good when I used a proper cord

The fixture looks like a C and M Products/Thomas Lighting design. Ballast dates to November 1969 BTW, almost 50 years old.
