I have come across a few of these NIB in the green Energy Choice packaging of the early-90s. I remember when they were common on store shelves, but I was more interested in circline adapters at the time.

I believe these are later ones, packaged and sold mostly for commercial use. For consumer use, I think electronically ballasted Biax adapters were the norm at the time these were made.
