This is what was lurking behind the handhole door that was falling off of a light post at a school which I have fortunately graduated from. The mixing of yellow (277V/480V) and the other colors (120V/208V) is a no no, but the idiots responsible for other monkey work that I saw there are to be expected of this.

I've seen worse there, as that photo I posted of the receptacle cover falling off the raceway was at the same place. There were also NEMA 1 (indoor dry location only) boxes outside, wiring hanging out of holes cut in fresh drywall, you name it. At least it wasn't like the LEED platinum building I saw once that had no insulation and depended upon dozens of electric plug in space heaters to keep the building even remotely habitable. Because the HVAC system did not work.
