F40T12 Instant start

Another instant start F40T12 bipin fixture. Who made this one? The ballast is twice as wide as a typical rapid start for the F40T12. This one worked for about 30 minutes then decided to quit. Ballast made by Consolidated electric corp.

Did you shunt the pins on the tube, at least on the end where the black and white wires are?

Yes pins on tube were shorted. Only the white wires hit the sockets on the one end and green and red on the other. The smell and the smoke coming from the ballast sort of gave it away... The one bulb glows very brightly and the other normally. Didnt put a meter on it because it was pretty bad smelling at that point.

Oh yea that is definitely a problem. I have had F96 ballasts do similar. Leaving a tube in place that is flashing/rectifying will kill those ballasts.
