Nice find! I think it includes shocker+starter.

It is a rapid start ballast, which are unique to North America. It works by providing a voltage that can only strike the lamp when the cathodes are heated, so the lamp flickers at startup while the cathodes heat up.

Don't be fooLED, T8 IS the worst thing to do to a magnetic T12 fixture.
