I've got a Philips 13w PL-C lamp with a small hole in the phosphor like that. Maybe its caused by some contaminant on the glass preventing the phosphor from sticking there?

It’s definitely not loss of the lamp’s seal. I’ve managed to light it before. Also, the phosphor hasn’t completely cleared from one end like it normally does when the seals go bad.
