Got it from an old Sylvania Alleykat fixture along with an Advance ballast and ignitor, A General Electric capacitor and an Area Lighting Research button type PC dated January 1984

Definitely older - the current Sylvania stock do not have a dimple at the top (probably why with the currently manufactured lamps, the arc tubes get bent out of shape), and the wire to the top electrode curves around the arc tube.

This is highly likely from 1984 too, it has the early 80s design frame work....I know how to date code the Sylvania lamps, give me the number that's found I believe on a metal strip inside....usually they start out with "SR___" and the number....if there aint on strip, then it may be on other part of the lamp...
