This is an Advance LQ-106F-TP magnetic rapid start ballast for 1 F4T5, F6T5, or F8T5 lamp. These are rarer than the 2 lamp version and have more of a startup flicker (Tested with a near EOL Chinese GE F6T5/CW lamp).

Wow awesome! The closest I have is a preheat F13T5 ballast with an internal electronic starter. Basically a magnetic "programmed start" ballast.

Remember wattage of the lamp really means nothing! The ballast is going to run lamps at the power it(ballast) was designed for.
Running a lamp other than those listed will have unknown results.

OCV may not be high enough for it to strike, and you'd need to know the total length of that CFL (as if it was a straight tube)

Colored Fluorescent's such as F40T12 Red or  Green or Blue are awesome...
