My almost unused desk lamp came with 2 Sylvania warm white lamps with this style etch and the best guess would be mid 1950s or a tad later ..

I'm looking at the lamp ordering code F15T8/CW which debuted on the etches of USA made lamps in 1958-1959. This lamp does not have the classic line bisected by a "/" etch used from 1960-1980. I see a year dash over "15W" which tells me this lamp is from the time period mentioned above. I see what may be a third dot faintly under the "A" in "preheat" which make this from 1958. If that's just a speck of dirt then this lamp is from 1959.
