The crews that took the old telegraph/telephone poles down were in a hurry and many of the cross arms fell apart and landed with insulators on them. Many were never picked up, especially out in the countryside.
I saw a few collapsed poles with insulators on the side roads.

WSDOT used a few of these. There's a section of these on I-405 near Bellevue. A lot of them on WA-161 in northern Puyallup, and a few on I-90 in Cle Elum and Ellensburg.

Oddly enough, the was a single drop lens M-400R2 on that highway but they seemed to have used a mix of GE luminaires. There was a bunch of M-400A2 FCO lights too.

These fixtures are becoming pretty rare. Back in the 2000s, SoCal was covered in these fixtures, particularly along the freeways and at signalized intersections installed in the early 90s.

However, they seemed to have a lot of issues. One problem is the glass tended to “fog up” significantly and the fixtures seemed dim as a result. They seemed to not really hold up as well compared to the M-400A2 FCOs. Norwalk, CA used to be lined with hundreds of these, and the majority were systematically replaced in the mid 2010s with newer GE M-400 FCOs. But, it’s still really neat to see one of these hanging around.
