I am not sure if this one was made by motorola, then labeled sylvania after they bought motorola in 2003, but i have never seen a ballast that powers this type of lamp, and im guessing all 3 are in series?

Yep, that's a Motorola ballast for 40W single-ended twin tube T5
The old part # is M3-PN-TT5/40-B-120
The Motorola datasheet says "Series wired programmed rapid start lamp operation" and it has lamp EOL protection.
