There's also a Form 402 , an admiral's hat and a top mount Form 400 incandescent . Not shown but appparantly , there was also radial waves there .

The Form 402 is simply a Form 400 for 700/1000w lamps , hence the longer "neck"  The Form 400 Powerpack has a WAY longer neck for the ballast/caps.  : https://www.lighting-gallery.net/gallery/displayimage.php?album=search&cat=0&pos=5&pid=236149
