I sure have. A number of Chicago’s original HPS Crimefighters from the early 70s didn’t have them.

Same here. My GE HM1000 doesn't have a NEMA tag either. It is from 1977 so it should technically have one, but it doesn't.

My area adopted HPS for new installs pretty early on (first install late 60s, consistently HPS by mid 70s) so a lot of the older HPS installs with GE, Westinghouse, MC-Graw Edison luminaires did not have NEMA tags.

Lots of older Powerlite/Sylvania luminaires did not come with factory NEMA tags as well. I think they only starting putting them on in the mid 80s or so.
