A word of warning.
The seller listed below seller does not pack their bulbs very well and you may end up with broken bulbs during shipping.

One F40T17 Sylvania white = Broken
One F72PG = Broken
One F48PGWM = Broken
They did refund my money, but that don't make up for the very poor packaging and the loss of such rare bulbs.
It is easy to see why the shipping is only $4.99

Heh. I have several cases of F15T12s that I am running in T8 ballasted fixtures. They last longer than the T8s do. I love how both ends black up on the T12s too in these T8 fixtures.
